Bill of Rights and Responsibilities


As a patient of Empowerment and Recovery Psychotherapy, patients have the right and responsibility to:

  1. Be treated with dignity and respect and treat providers with dignity and respect
  2. Reasonable access to care regardless of race, ethnicity, gender identity and expression, sexual orientation, religion, age, or ability differences
  3. Ask the provider about work history, professional training, and credentials
  4. Know the terms of therapy such as cost, payment method, and the provider’s cancellation policy
  5. Have treatment and other information kept confidential. Records may be released without permission only when permitted by law
  6. Have all things explained in terms that can be understood and to have any questions answered concerning diagnosis and treatment
  7. Collaborate in decision making regarding treatment
  8. Refuse psychological or medical treatment recommendations. If treatment is refused, patients have the right to an explanation of the consequences of non-adherence to treatment
  9. Participate in the assessment of ethical concerns that may arise during the provision of care
  10. File a grievance should a dispute arise over the delivery of care
  11. Ask questions about the provision of care
  12. Understand their health care coverage and plan options
  13. Provide accurate information when requested to support the delivery of appropriate care
  14. Tell their provider about medication changes, including medications given to them by others
  15. Collaborate with the development of treatment goals
  16. Pay necessary fees at the time of service
